A life insurance and accidental death and dismemberment policy is provided and fully paid for employees (working 17 hours or more per week) with coverage available on the first month after an employee has completed three months of employment. The amount of coverage will be the annual wages or salary rounded up to the nearest $1,000, up to a maximum of $50,000. The annual wages are calculated by multiplying base hourly wage times standard weekly work hours times 52 weeks in the year. Employees are required to complete a Beneficiary Designation form upon hire and can read the Summary Plan Booklet for:
